How to Apply
Application complete in all respect alongwith relevant testimonials, certificates (Attested) and two recent passport size photographs should be sent to Director General Health Services, Haryana, Sector-6, Panchkula. One photograph should be pasted on right hand top corner at the space provided in the application form duly attested by a Gazetted Officer and second be attached with the application from. Application complete in all respects should be sent in a cover superscribed “Application for the post of Medical Officer” to reach the following address on or before the closing date i.e. 30.09.2010.
